International Cosmos Prize 2020 Web Recommendation (CLOSED)

■You can recommend candidates for the 2020 International Cosmos Prize by submitting either a handwritten or typed
form.

To use the former, please complete the provided recommendation form by hand and return it to use in the self-addressed envelope. To use the latter, you may select method A or B described below.

A.Web Recommendation Form:

　Please fill in the Web Recommendation Form and check that it has been completed.
　You can submit the form with just one click. Please note, however, that you cannot save the draft date on the web.

B.Recommendation Form(in Microsoft Word format)

　After downloading the file for the Recommendation Form in Microsoft Word format,
　please send the completed file by attaching it to an email message.

To use the Web-based Recommendation Form or to download the Recommendation Form in Microsoft Word format, you need to enter the password indicated in the written request for recommendations that was sent to you.
Only members who have been selected by the International Cosmos Prize Committee as recommenders are qualified to recommend candidates for the prize.
The Microsoft Word file can be submitted either by attaching to an email message, or by printing it out and sending it by postal mail,to the Secretariat of the INternational Cosmos Prize Committee (at the address below).
